March 13, (THEWILL) — The Nigerian Safety Investigation Bureau (NSIB) has released the preliminary report on its ongoing investigation into the serious incident involving a Boeing 737-700 aircraft with nationality and registration marks 5N-MJF, operated by Arik Air Limited.
The report issued on Friday, signed by the Bureau’s Director, Public Affairs and Family Assistance, Mrs Funke Arowojobe revealed that, on 26 August 2021, ESN 805612 was installed at the number 2 position on an aircraft with power assurance check, by Israel Aerospace Industries (IAI) Ltd., BEDEK Aviation Group.
The Bureau also reported that the aircraft had a valid Certificate of Airworthiness and that, the flight crew were licensed to conduct the flight.
While climbing through FL 260 to FL 270, the flight crew heard a loud bang and the First Officer was the Pilot Flying while the Captain was the Pilot Monitoring.
On wreckage and impact information, NSIB disclosed that the engine inlet cowling was missing.
The Integrated Drive Generator (IDG) oil cooler was damaged, and the anti-ice duct was broken, and part of it was missing.
Also, the engine oil tank filler cap was missing while the Outlet Guide Vanes (OGV) at 5, 6, and 7 o’clock positions were missing.
It added that there were cuts on the left wing leading edge and also visible damage on the exhaust assembly.
It stated, “The occurrence took place on 11 February 2026 during a scheduled passenger flight from Murtala Muhammed International Airport, Lagos, to Obafemi Jeremiah Awolowo International Airport, Port Harcourt.
“During the flight, the crew reported an abnormal engine event, which prompted emergency procedures and a diversion to the nearest airport- Benin. The aircraft landed safely, and all occupants onboard disembarked without injuries.
“The preliminary report presents the factual information gathered so far, including details from the flight crew, air traffic control communications, flight recorders, and initial examination of the aircraft.”
The Bureau further reiterates that the investigation remains ongoing, and further technical examinations are underway.
“The preliminary report is available for download on the NSIB website for members of the public and the media.
“The Bureau notes that the information contained in the preliminary report is subject to further review and analysis. The final report will present the Bureau’s conclusions and safety recommendations,” NSIB added.
